Due to unforeseen circumstances, we are re-advertising this exciting opportunity for an enthusiastic, motivated Pharmacy Technician to join our innovative, award-winning pharmacy team.

Based across South Kerrier, we provide care for over 33,000 patients across five practices: Helston Medical Centre, Mullion & Constantine Group, Cober Valley Health, St Keverne Health Centre, and St Marys Health Centre, Isles of Scilly. Our pharmacy team is highly valued, well integrated and respected across the PCN and member practices.

Our established team includes a Senior Lead Clinical Pharmacist, Senior Audit & Systems Pharmacist, Clinical Pharmacist and three Pharmacy Technicians. We have a strong training ethos, supporting pharmacy learners and the wider multidisciplinary team. Our recognised audit programme has received local and national acclaim for improving patient safety.

We are a friendly, supportive team that values development, offering regular team meetings (with cake!), monthly peer review, mentoring and clinical supervision.

General practice pharmacy is varied and rewarding, with no two days the same. We offer excellent training, ongoing senior and technician support, and opportunities to develop specialist interests through structured project work in a supportive learning environment.

Job Purpose

The Pharmacy Technician will work as part of a multidisciplinary team within the Primary Care Network (PCN) to support medicines optimisation, improve patient outcomes, reduce medicines waste, and contribute to the safe and effective use of medicines across member practices.

The post holder will support clinical pharmacists, GPs, nurses, and other healthcare professionals by undertaking medicines reconciliation, medication reviews, repeat prescribing support, clinical audits, prescribing safety work, and care home medicines optimisation.

The role aligns with the objectives of the Additional Roles Reimbursement Scheme (ARRS) and NHS Long Term Plan.

Key Responsibilities

Medicines Optimisation

Support the delivery of medicines optimisation initiatives across the PCN.
Identify patients who may benefit from medication reviews.
Improve medication adherence through patient education and support.
Assist in the implementation of local and national prescribing guidance.
Support the management of medicines safety alerts and recalls.
Medicines Reconciliation

Undertake medicines reconciliation for patients following:
Hospital discharge
Outpatient appointments
Transfers between care settings
Resolve medication discrepancies and ensure accurate clinical records.
Liaise with hospitals, community pharmacies, care homes, and other healthcare providers.
Repeat Prescribing Support

Process repeat prescription requests within agreed protocols.
Identify inappropriate requests and refer where necessary.
Support synchronisation of medicines and reduce unnecessary prescribing.
Contribute to reducing medicines waste and improving prescribing efficiency.
Long-Term Condition Support

Assist with monitoring patients receiving high-risk medicines.
Support clinics relating to long-term conditions such as:
Diabetes
Hypertension
Respiratory disease
Cardiovascular disease
Ensure monitoring requirements are completed and recorded.
Care Home Medicines Optimisation

Support medicines management within care homes.
Conduct medication reviews and audits under appropriate supervision.
Work with care home staff to improve medicines administration processes.
Assist in reducing medication-related risks.
Quality Improvement and Audit

Participate in clinical audits and quality improvement projects.
Analyse prescribing and medicines-related data.
Support QOF, IIF, and other contractual requirements.
Assist with service evaluations and reporting.
Patient Engagement

Provide advice regarding medicines and their safe use.
Respond to routine medicine queries.
Support shared decision-making and self-management.
Refer complex clinical issues to a pharmacist or GP.
Working Across the PCN

Work across multiple practices within the Network.
Develop effective working relationships with practice teams.
Participate in multidisciplinary team meetings.
Support integrated working across primary, community, and secondary care.
Information Governance

Maintain accurate records using approved clinical systems.
